Payment Unsuccessful

Your one-time payment was not successful. As a result, your Notice of Election to be Exempt was not submitted to the Division of Workers’ Compensation. If you wish to submit a Notice of Election to be Exempt for the construction industry, an exemption fee of $50 plus a convenience fee of $1 must be made by Visa, MasterCard, Discover, American Express (prepaid cards are acceptable), or from a business or personal checking or savings bank account. In order to submit a new exemption application and the required processing fee, click on the Restart Application button below and then click on Apply for or Renew an Exemption button.
