If you choose to delete
a self insurer An entity licensed by OIR under Chapter 624 (p. III) or Chapter 641 (p. I) of the Florida Statutes., all closed claim A demand for payment from a self-insured entity or an insurer for losses sustained by a claimant. or aggregate reports associated with
that insurer will be deleted.
Setting Up a Self Insurer
This page will walk you through adding a self insurer to use for your
closed claim or aggregate report. If you do not have a self insurer your
Workbench, the screen will appear as it does below.

To Add a Self Insurer to Your Workbench
If you
wish to add a self insurer to your Workbench:
Click the Add
new self-insured An individual or entity that has not purchased liability insurance. entity button. The Add Self-Insured Entitypage will display.
The Add Self-Insured Entity Page
Fill in the following
fields as appropriate:
Type
Type indicates whether the insurer is an individual
or entity. Select
Individual or Entity from the drop down box as appropriate.
First Name
The given name of the insurer if the Type is
Individual.
MI
The middle name of the insurer if the Type
is Individual.
Last Name
The last name of the insurer if the Type is
Individual.
Entity Name
The name of the insurance company if the Type
is Entity.
License Number
Also known as the registration number, this
is the official number capturing an attorney or physician's authorization
to practice. Note
that the last two-digits of the license number are for the profession
code.
FEIN Number
The company's nine-digit Federal Employer Identification
Number (FEIN). You can enter the FEIN with or without the dash. For example,
you can enter '59-2238371' or '592238371' to retrieve the company 'Ace
Finance Corporation'.
Self Insured Entity Type
This field describes the type of self insured
entity. Self
Insured Entity Type includes Attorney, Medical Practitioner or Hospital.
Click the Save
button. The
self insurer you created will be added to your Workbench and will now
appear on the Self Insured Entity page.

Deleting Self Insurers
You may delete a self insurer you no longer
provide services for or accidentally added to your Workbench. However,
please note that if you choose to delete a self insurer, all closed claim
or aggregate reports associated with that insurer will be deleted. PLCR
will warn you of this fact and ask you to confirm the deletion if you
have closed claim or aggregate reports associated with that self insurer.
To Delete a Self Insurer
Click the radio button next to the name of the
self insurer you wish to delete from your Workbench.
